1. Voluntary participation. I have voluntarily chosen to participate in training in the sport of karate at Ascent Dojo, operated by Evangelista Martial Arts Inc. ("Ascent Dojo"), a dojo affiliated with the International Karate Organization Kyokushinkaikan (Canada) ("IKOK-C").

2. The activities. Kyokushin karate is a martial art that involves full physical contact. As part of this sport I may be directed to participate in:

• physical warm-ups that may involve stretching, calisthenics (including push-ups and sit-ups), running and other training methods;
• karate exercises (kihon) that involve kicking, punching and blocking;
• movements known as kata, which are combinations of karate forms;
• self-defence, which may involve joint manipulation, various holds on all parts of the body (including the neck) and break falling;
• sparring and fighting (kumite), which all of these exercises lead to and which are integral parts of Kyokushin karate.

3. Risks. I understand that there are inherent physical risks involved in these activities. All of them carry a risk of physical injury or death, as contact is applied on both the receiving and attacking end. I understand and accept these inherent risks.

4. Directions and rules. I agree to follow all directions and warnings given to me by the instructors of the dojo, and to wear the protective equipment my instructor requires. I understand that my failure to follow such directions or warnings may result in physical injury to myself or others. I agree to obey all the rules, policies and regulations governing the operation of the dojo as advised to me by my instructor, posted at the dojo, or set out on the Ascent Dojo or IKOK-C websites or in the Student Handbook.

5. Release and indemnity. I hereby indemnify and hold harmless the Released Parties against all losses or claims of whatever kind in connection with loss of life, personal injury, economic loss or damage to property arising from my participation in this sport, including losses or claims caused by the negligence of the Released Parties. I agree not to make any claim or take proceedings against the Released Parties for any losses or claims of whatever kind arising from my participation in all of the activities of the dojo, including the regular classes and special events such as tournaments, gradings, camps and seminars that I may become involved in.

6. Released Parties. The "Released Parties" are Evangelista Martial Arts Inc., operating as Ascent Dojo, and its owners, directors, officers, employees, Head Instructor, instructors, volunteers, members and students; and the IKOK-C, the Board of Directors and Officers of the IKOK-C, the IKOK-C Branch Chief, and all instructors and members of the IKOK-C.

7. Students under 19. If the student is under 19 years old, I am signing as the student's parent or legal guardian. I understand the risks described above, I have explained them to the student, and I consent to the student's participation. I agree to these terms on the student's behalf and on my own behalf, to the fullest extent permitted by law.

Part 2: Photo release and signature

PHOTO AND VIDEO RELEASE (OPTIONAL)

I give permission for photographs and video of me (or of the student, if I am signing as parent or guardian) to be taken and used by Ascent Dojo and/or the IKOK-C in brochures, websites, social media, leisure guides and other promotional materials that Ascent Dojo and/or the IKOK-C create, including permission for Ascent Dojo and/or the IKOK-C to copyright the photographs and video in their names. The purpose of the promotional materials is to encourage people to participate in the activities of Ascent Dojo and/or the IKOK-C.

If I check the box above, I release Ascent Dojo and the IKOK-C from all claims arising out of their use of the photographs and video, including all claims for libel or invasion of privacy. I may withdraw this permission for future use at any time by notifying Ascent Dojo in writing. If the box is left unchecked, no permission is given.
